I. Park Card Applications for Owners and their Families
Step A.
Complete the Application for Park Card and provide all proof as required in the application.
Step B.
Complete one of the Homeowner Affidavit Forms:
Homeowner Affidavit Form
Permanent Resident Affidavit Form
For Tenants Only – Tenant Affivadit Form
Step C.
Send all materials to Park@villageofeasthills.org.

- Park Cards for Returning Family Members :
- Apply for Park Card in Step A above
- Fill out the Affidavit for Returning Family Members – Homeowners, and
- Have each returning family member over 18 fill out the Affidavit for Returning Family Members – Family Member
